- The fourth trimester refers to the first 12 weeks after birth — a critical period of physical and emotional recovery. ACOG recommends ongoing care throughout this period rather than a single 6-week visit, including a 2-week contact for all patients.
- The 6-week postpartum visit (~42 days after birth) includes a pelvic exam, C-section wound check if applicable, contraception discussion, PPD screening using the Edinburgh Postnatal Depression Scale (EPDS), and clearance for sexual activity and exercise.
- Vaginal births: gentle walking 1-2 weeks; moderate exercise after 6-week clearance; high-impact activity (running) from 12 weeks. C-section: 6-8 weeks for moderate activity; 12+ weeks for high-impact. Pelvic floor physiotherapy assessment at 12 weeks is recommended.
